Used to have a copy long time ago. Have since lost it. Anybody know where I can find a copy, either hardcopy or online?

It's definitely a hard one to track down these days. I don't think a
scan of it has ever ended up online. A few years ago I saw a reference
to it in another book or article I was reading and got curious, so
started keeping an eye out for a copy to pop up on AbeBooks or eBay.
Eventually there was a pretty beat-up copy that popped up on eBay and I
snagged it.

So copies may pop up now and then if you keep an eye out.
