Some books have index main entries with so many sub-entries that they continue on the next page of the index. Suppose an index contains
letters
a, 10
b, 20
c, 30
d, 40
but the sub-entry b, 20 is at the bottom of a page. A user-friendly book would start the next page of the index with

letters (continued)
c, 30
d, 40

but how I get that effect without knowing where the page break will be?

An old package does the trick, "\usepackage{repeatindex}", but note that it was one of a very old package, and doesn't have any support and maintenance.
