Summary description: Macros for setting numbered entries in shuffled order

Minor bugfix update:

Fixes some rare unhandled edge cases where the use of multiple
fixed-index entries could cause some of those entries to become
shuffled to pseudorandom indexes instead.

Also fixed and documented 4 macros, and removed an unneeded temporary
macro.
